2. Complete the LPN-RN Transition Program application along with the checklist and submitted with Official Transcript to the Office of Admission at 1073 W. Broad St. Suite 201, Falls Church VA 22046. Payment plan: 50%  down of the total Semester cost 7 calendar days prior to start of each Semester and Balance 50% paid 7 calendar days prior to the start of the 2nd 8 weeks of  each Semester.
